CNOR Exam Format: 200 Questions in 3 Hours 45 Minutes, the Subject Areas, and the 2027 Outline
The CNOR has 200 multiple-choice questions, 185 scored and 15 unscored pretest items, in 3 hours 45 minutes at a PSI test center or remotely. Here is how many scored questions each subject area carries, how to pace the sitting, and what CCI has published about the six-area exam planned for 2027.
Short answer
The CNOR exam has 200 multiple-choice questions and a 3-hour-45-minute time limit. 185 questions are scored and 15 are pretest questions that don't count and can't be identified. You take it by computer at a PSI test center or as a remote proctored exam. The scored questions follow seven subject areas, led by intraoperative patient care and safety (46 questions) and infection prevention and control (30). CCI plans a six-area exam for 2027 with the same 200 questions and time, but hadn't published its launch date as of October 27, 2026.
The CNOR is one fixed-length sitting: 200 multiple-choice questions in 3 hours 45 minutes, on a computer. CCI's CNOR handbook (version 5.2026) says 185 of the 200 questions are used to calculate your score and the other 15 "serve as pre-test questions and do not affect your score." Those 15 are "dispersed throughout the exam and cannot be identified."
The CNOR exam at a glance
As checked on October 27, 2026:
| CNOR exam | |
|---|---|
| Questions | 200 multiple-choice |
| Scored | 185 |
| Unscored pretest | 15, mixed in and unidentified |
| Time limit | 3 hours 45 minutes, kept by the testing computer |
| Delivery | Computer-based, at a PSI test center or as a Remote Secure Proctored Exam |
| Tutorial | Optional, before the exam, not counted in your exam time |
| Scoring | One pass or fail decision on a scaled score from 200 to 800; 620 passes |
The tutorial shows how to select answers and mark questions you want to return to before you finish. CCI also advises answering every question, because "any question not answered may count against the final score." The CNOR passing score guide explains the scale.
If you see a description of 170 scored and 30 unscored items, it doesn't match CCI's current handbook.
CNOR subject areas and how many questions each carries
The handbook's "Subject Areas on the Exam" table assigns all 185 scored questions:
| CNOR subject area | Share | Scored questions |
|---|---|---|
| 1. Pre/postoperative Patient Assessment and Diagnosis | 15% | 28 |
| 2. Individualized Plan of Care Development and Expected Outcome Identification | 8% | 15 |
| 3a. Management of Intraoperative Activities: Patient Care and Safety | 25% | 46 |
| 3b. Management of Intraoperative Activities: Management of Personnel, Services and Materials | 9% | 17 |
| 4. Communication and Documentation | 11% | 20 |
| 5. Infection Prevention and Control of Environment, Instrumentation and Supplies | 16% | 30 |
| 6. Emergency Situations | 10% | 19 |
| 7. Professional Accountabilities | 6% | 10 |
CCI counts seven subject areas, with area 3 in two parts; its CNOR page shows the same weights as eight rows. Together, the two parts of area 3 are 34% of the exam, 63 scored questions. Add infection prevention and you have almost exactly half the exam (93 of 185, Passy's arithmetic).
What each area covers is spelled out in CCI's Task & Knowledge Statements (updated 12/2023). Each task, such as "Perform appropriate surgical counts" or "Adhere to appropriate procedures for sterilization, biological monitoring and chemical monitoring," is followed by the knowledge it draws on. CCI notes the statements "may not reflect all specific tasks" in niche or highly specialized settings. To work through them area by area, planned CNOR practice is tagged to both parts of Management of Intraoperative Activities and every other area.
What CNOR questions look like
Every CNOR question is multiple choice. The handbook includes five sample questions, each with four answer choices, and says they "represent similar content" to the exam and are meant to show the format, "not intended as study questions." They mix straight knowledge, such as anatomy or a normal value, with applied judgment about planning and assessment. CCI's handbook says the exam "assesses a combination of experiential and cognitive knowledge."
Passy doesn't reproduce CCI's samples. You can read them in the handbook, and CCI offers five practice questions with rationales on its practice questions page after a short form.
Pacing the CNOR: 200 questions in 225 minutes
Passy's arithmetic for an even pace:
| Checkpoint | Time used |
|---|---|
| Question 50 | about 56 minutes |
| Question 100 | about 1 hour 53 minutes |
| Question 150 | about 2 hours 49 minutes |
| Question 200 | 3 hours 45 minutes |
That is about 67 seconds per question. Mark questions you're unsure of and keep moving; the tutorial shows how to return to them.
Breaks depend on where you test. At a PSI test center, the handbook gives "one 10-minute unscheduled break." Remote candidates "are unable to take a break." Not returning from a break in time forfeits the attempt. The handbook doesn't say whether the exam clock pauses during the test-center break, so plan as if it doesn't. When you're ready to practice that rhythm, you can rehearse the 3-hour-45-minute sitting with the planned CNOR mock exam.
CNOR at a PSI test center vs remote
Both options deliver the same exam. The practical differences, from the handbook's PSI guidelines:
- Test center: Monday through Saturday by appointment, lockers for wallet and keys, video surveillance, one 10-minute break, military ID accepted
- Remote: 24/7 scheduling based on availability, a private room (CCI recommends a closed room with a door), a cleared desk with only your computer, mouse and keyboard (no extra monitors), a photo ID check, a selfie and room scans, no breaks, and no military ID
- Remote technology: PSI's secure browser isn't supported on work devices, tablets, hybrid devices, mobile devices, Surface Pros or Chromebooks. Run PSI's compatibility check at least 24 hours before your appointment
If you need breaks, the handbook's advice is to test in person. Booking, late-arrival and rescheduling rules are in the CNOR eligibility and application guide.
The CNOR outline planned for 2027: six competency areas
CCI's Certification Council adopted the results of the 2024 Job Task Analysis on November 7, 2024. CCI's job analysis page says the new CNOR exam will launch in 2027 with six competency areas, and that the new exams "remain unchanged in terms of the number of items and examination time permitted." The published item counts are:
| 2027 competency area | Scored questions |
|---|---|
| 1. Individualized Plan of Care Development | 42 |
| 2. Intraoperative Patient Care and Safety | 49 |
| 3. Management of Personnel, Services, and Materials | 24 |
| 4. Communication and Documentation | 20 |
| 5. Infection Prevention and Control | 33 |
| 6. Emergency Identification and Management | 17 |
The six add up to 185. CCI's crosswalk explains the changes: Pre/postoperative Patient Assessment and Diagnosis and Professional Accountabilities stop being separate areas and are folded into others, Management of Personnel, Services and Materials becomes its own area, and the other areas are renamed and condensed.
What CCI's job analysis FAQ says about the switch, as of October 27, 2026:
- No date yet. "The date that the new examinations will become available will be shared well in advance"
- No overlap and no gap. The two versions won't be offered at the same time, and there will be no period with no exam available
- Your test date decides the version. Candidates testing before the implementation date take the current exam. A retake after that date is on the new version
- Approved but not yet tested. Your eligibility generally stays valid, but once the new exam is implemented, that's the exam you take
- Study materials. CCI says updated materials and sample questions will be released before the new exam
Until CCI announces a date, the current seven-area outline is the one to study. Planned CNOR prep follows CCI's current subject areas and will re-map when CCI dates the new exam.
