CST Renewal: 30 CE Credits Every 2 Years, the $40 Fee, and What Happens If You're Late

A CST renews every 2 years with 30 AST-approved continuing education credits, 4 of them live, and a $40 NBSTSA application, or by passing the exam again for $540. Here is the two-step process, a timeline worked back from your expiration date, the 90-day late window, and the fees, as checked on October 28, 2026.

Short answer

A CST renews every 2 years. NBSTSA's renewal page, as checked on October 28, 2026, requires 30 AST-approved continuing education credits earned within your current cycle, including 4 live credits. You submit them to AST first, then file NBSTSA's renewal application with a $40 fee, as early as 6 months before expiration. If you are 1 to 90 days late, you can still renew for $140 in total, but only with credits earned before you expired. Otherwise you renew by passing the CST exam again, for $540.

Renewing a CST is a two-step process with two organizations. You earn and submit continuing education (CE) credits to the Association of Surgical Technologists (AST), and only after AST approves them do you file the renewal application with NBSTSA. The gap between those steps is where a renewal can slip past its deadline, so the timing matters as much as the credit count.

Step 1: earning and submitting CST CE credits through AST

NBSTSA's renewal page sets the rules for the credits themselves:

  • Earn them inside the cycle. Credits earned before your cycle started or after it ended don't count.
  • Use AST-approved activities. NBSTSA points to AST's CE Resource Center for eligible activities and says CE not approved by AST won't be accepted.
  • Get your live credits. NBSTSA counts AST-approved conferences, state assembly meetings, webinars, workshops and other in-person or in-service events as live CE.
  • Submit on AST's CE reporting form. NBSTSA says a fee applies for credits earned during periods when you weren't an AST member.
  • Allow time. NBSTSA says AST needs 6 weeks to process CE credits, and it recommends submitting them 60 days before your expiration date.

The AST reporting form posted on NBSTSA's site (revised April 2022) adds working detail: one CE credit equals 50 to 60 minutes of activity, partial credit starts at 30 minutes and grows in 15-minute steps, and the form listed a $400 processing fee for nonmembers. The form says prices are subject to change, so check AST's current fee before you submit.

Step 2: the NBSTSA renewal application

Once AST approves your credits, log in to NBSTSA's Learning Builder platform, which uses the same login as AST, complete the renewal application and pay the $40 fee. NBSTSA also accepts a mail-in renewal application, and it holds check payments for 10 business days while they clear.

NBSTSA notes that certification doesn't renew instantly, because AST's CE processing comes first. While you wait, NBSTSA can't email or fax your employer proof that a renewal is pending, though it can confirm by phone that it received your application.

A CST renewal timeline, worked back from expiration

A worked example (fictional CST, Passy arithmetic). Sam's cycle expires on December 31, 2027.

DateWhat happens
Throughout the cycleSam earns 30 AST-approved credits, at least 4 live, all before December 31, 2027
July 1, 2027About the earliest Sam can renew by CE, 6 months before expiration
By November 1, 2027NBSTSA's recommended date to submit CE: 60 days before expiration
About mid-December 2027AST's 6-week processing ends for credits submitted on November 1
Before December 31, 2027Sam files the NBSTSA application and pays $40
January 1 to March 30, 2028Late window, 1 to 90 days after expiration: $40 plus a $100 late fee, with credits already earned by December 31
From March 31, 2028Renewal by exam only

The lesson in the middle rows: if Sam waits until December to submit CE, AST's processing time alone pushes the NBSTSA step past the expiration date. Submitting in the summer or early fall leaves room for a returned form.

Late CST renewal: the 90-day window

NBSTSA's policies are specific about late renewal:

  • It doesn't add time to earn credits. All required credits must have been earned while you were certified, before your expiration date.
  • It costs $140 for a CST. That is the $100 late fee plus the $40 renewal fee, for applications filed 1 to 90 days after expiration.
  • It keeps your record intact. If you renew within the 90 days, you keep your certification number and get a new expiration date as though you had renewed on time.
  • After 90 days, or without the credits, the exam is the route. NBSTSA's FAQ puts it briefly: everyone outside that window must test.

While a certification is lapsed, remember that NBSTSA's handbook ties use of the CST initials to keeping the certification current.

Renewing the CST by exam

You can renew by passing the national CST exam instead of submitting CE, for $540, a fee that includes NBSTSA's $100 non-refundable processing fee. NBSTSA's eligibility policy lists current or previously certified CSTs renewing by examination as an eligible group, and the renewal application includes a "recertify by examination" box. It's the same exam first-time candidates take, with the same 98-of-150 passing standard. If you are weighing that route, full CST prep planned for launch follows the same outline a renewal candidate faces.

NBSTSA's own position favors CE. Its policies say it believes renewal should come through professional development and continuing education rather than an entry-level exam.

For a CST who has been in one specialty for years, the hardest part of a renewal exam is often the breadth: other specialties, Basic Science and dose limits. The planned CST flashcards cover instruments, sutures, hemostatics, wound classes and maximum doses, which is the recall load a long-practicing CST may need to rebuild. If you fail a renewal attempt, the retake rules in the failed CST exam guide apply: no waiting period and up to three tests per calendar year.

The wallet card, certificate and name change details come from NBSTSA's card request page, which notes that NBSTSA no longer mails printed certificates. The score transfer fee is on NBSTSA's score transfer form. AST's CE processing fees are separate and set by AST.

Dual CST and CSFA holders, and CSTs certified before 1977

  • CST and CSFA together. NBSTSA's renewal page lists 38 credits in total per 2-year cycle and says to submit the CSFA form only.
  • Certified before September 1, 1977. NBSTSA's history page says the renewal requirement applies to CSTs certified after August 31, 1977. The 2026 handbook says earlier certificants can become current, with an expiring certificate and number, by submitting 30 CE credits earned in the two years before applying or by passing the exam. NBSTSA's initial currency application refers to 60 credits processed by AST, so call NBSTSA at 800-707-0057 to confirm the number before you start.

Older CST renewal information you may still find

You may still see a four-year cycle with 60 credits described elsewhere, including on the AST reporting form (revised April 2022) that NBSTSA links, which lists both a four-year and a two-year cycle. NBSTSA's current renewal page lists only the 2-year cycle with 30 credits, and NBSTSA's page on how it sets the standard states the same: 30 credits every two years, at least 4 of them live. Go by the cycle dates on your NBSTSA record.

Proving your CST is current

NBSTSA's public verification lookup shows your certification number and cycle start and expiration dates. You can print your certificate free from your account at any time, and a wallet card is available as described above. If an employer needs proof while your renewal is processing, NBSTSA can confirm by phone that it received your application.

Your next step for CST renewal

Find your expiration date in your NBSTSA account and write down three dates: 6 months before it, 60 days before it, and the 90th day after it. Then count the AST-approved credits you already have, including live ones. If you're short and the date is close, plan the live credits first, since they're usually the hardest to schedule. If the exam is your route back, compare the planned CST prep lengths before you book a recertification date; checkout is not open yet.
