MFT National Exam Format: 180 Questions, 4 Hours and What Counts Toward Your Score
The AMFTRB MFT National Exam has 180 four-option multiple-choice questions in 4 hours. In 2026 all 180 count, per AMFTRB's Exam Reference page; from January 2027, 155 are scored and 25 are unscored pretest items. See the domain item counts, the pace, how items look, and how breaks and the timer work.
Short answer
The MFT National Exam has 180 multiple-choice questions, each with four options and one correct answer, and you get 4 hours at a Prometric test center. The 2026 handbook describes no unscored items, and AMFTRB's undated Exam Reference page says all items count. From the January 2027 window, 155 are scored and 25 are unscored pretest items mixed in at random, so you can't tell which ones count. At an even pace, that's 80 seconds a question. There are no scheduled breaks, and the clock keeps running if you step out.

Sources: the 2026 handbook (180 multiple-choice questions, four hours, domain counts that add up to 180), the 2027 handbook (155 scored plus 25 pretest items, four hours) and AMFTRB's Exam Reference page, which carries no date and says each form "contains 180 items," that "all items are counted in computing the candidate's score," and that each item "has four alternative answers, only one of which is correct." The 2027 handbook is the first to describe unscored items; the Exam Reference page hadn't been updated to match when checked on October 27, 2026.
What the 25 pretest items mean for you from 2027
The 2027 handbook says the pretest questions "are randomly distributed throughout the examination and do not count towards a candidate's score." AMFTRB is trying them out to see whether they perform well enough to become scored items on a future exam.
In practice:
- You can't spot them. They are mixed in with scored items, so treat every question as if it counts.
- Your time budget doesn't change. You still answer 180 questions in 4 hours.
- Your score comes from 155 items. The handbook says "only the scored items count towards the candidate's final score."
The 2027 changes guide lists the other rule changes that start with the January 2027 window, including the fee and the two-week windows.
Questions per domain on the MFT National Exam
Both handbooks print the same blueprint, titled "Exam Blueprint Updated for 2026 Examinations," based on a 2024–2025 job task analysis. Only the counts change, because 2027 separates scored from pretest items:
| Domain | Share | 2026: items | 2027: scored | 2027: pretest |
|---|---|---|---|---|
| 1. The Practice of Systemic Therapy | 16% | 29 | 25 | 4 |
| 2. Assessing, Hypothesizing, and Diagnosing | 16% | 29 | 25 | 4 |
| 3. Designing and Conducting Treatment | 17% | 30 | 26 | 4 |
| 4. Evaluating Ongoing Process and Terminating Treatment | 16% | 29 | 25 | 4 |
| 5. Managing Crisis Situations | 16% | 29 | 25 | 4 |
| 6. Maintaining Ethical, Legal, and Professional Standards | 19% | 34 | 29 | 5 |
| Total | 100% | 180 | 155 | 25 |
The exam is unusually flat: five domains sit at 16% or 17%, and only ethics, law and professional standards stands out at 19%. No domain is small enough to skip. To practice in proportion, you can see how planned practice tags each question to one of the six domains, with the most questions in the ethics domain.
Pace: 80 seconds a question
Four hours is 240 minutes. Spread evenly over 180 questions, that's 80 seconds per question, or about 45 questions an hour (Passy's arithmetic; the handbooks give only the total time). A workable checkpoint plan:
| Elapsed time | Questions answered at an even pace |
|---|---|
| 1 hour | 45 |
| 2 hours | 90 |
| 3 hours | 135 |
| 4 hours | 180 |
Some questions are short recall items and some come with a case description, so your real pace will vary. The point of the checkpoints is to notice early if you're behind. AMFTRB's online practice exam uses the same rate, 90 questions in 2 hours, if you want to test your pace on official material.
What MFT National Exam questions look like
The handbooks include six sample questions with an answer key. Without reproducing them, here's what they show about the format:
- Single best answer. Every item has four options. AMFTRB's Exam Reference page says to "choose the single best answer"; no credit is given if more than one response is selected.
- Shared cases. Two of the samples refer to the same short case description, so expect some items that come in sets.
- Sequencing words. One sample asks what the therapist should do FIRST, which tests priorities, not just facts.
- Systemic framing. Several ask which statement or focus reflects a systems perspective, or name a structural therapy concept.
- A diagram. One sample asks you to read a family diagram from a structural perspective.
Scoring is number correct with no extra penalty for wrong answers. The Exam Reference page says it's "to the candidate's advantage to answer each item even when uncertain." Never leave a question blank.
How the session works at the test center
From the handbooks and AMFTRB's FAQ:
- Tutorial first. A short tutorial explains how to select answers and move between questions. The FAQ says most candidates take 5 to 10 minutes and that tutorial time doesn't count against your exam time.
- Move freely and mark items. The FAQ says you can skip forward or back, review questions at any point, and mark any question, answered or blank, for review.
- Scratch paper. The FAQ says candidates get two pieces of paper and a pencil, which you return before leaving. You can't bring your own papers, notes or references.
- No scheduled breaks. The 2026 handbook says no breaks are scheduled. Both handbooks say you may leave the room only for the restroom or your locker, and "the exam timer will NOT be paused."
- Locker limits. During an unscheduled break you may take only snacks, drinks, medicine or personal healthcare items from your locker. The 2027 handbook says accessing your phone for any reason ends the exam on the spot, without a score and with your fees forfeited.
- Don't exit by mistake. The candidate agreement says that once you exit the session, even in error, it can't be restarted; the exam is scored as is.
Rehearsing all of this at full length is the best way to find your pace; you can rehearse the four-hour sitting with the planned 180-question timed mock exam once full prep launches.
Testing time with accommodations or ELL extra time
The 4-hour limit can change in two ways, both requested when you apply and at least 8 weeks before your window starts:
- ADA test accommodations, requested from PTC with its official form and documentation.
- English Language Learner (ELL) extra time, 1 or 2 extra hours, available only if your state board allows it and approves you. The 2027 handbook lists fees of $85 for 1 hour and $135 for 2 hours on top of the exam fee, and warns that not every board accepts a score earned with ELL extra time.
The registration guide covers how to request both.
Using the format in your preparation
The format points to three habits: practice in mixed-domain sets, the way a full exam draws on all six domains; time yourself at 45 questions an hour; and read each stem through to its last line before the options, noting whether it asks for the FIRST step, the BEST explanation or a definition. The study guide builds those into a weekly plan, and the passing score guide explains how the scored items turn into a result.
Start with the 20 free MFT practice questions, each with an explanation. Twenty items can't tell you your score, but they show how case-based questions read. For more volume later, 540 flashcards sorted by domain are planned for quick recall, alongside the question bank.
