NBCOT OTR Exam Format: 180 Items, Scenario Sets, and the 4-Hour Clock
The OTR exam has 180 items in 4 hours: three- and four-option single-response questions mixed with scenario sets of four six-option items where you pick three. Here is how each item type works, how credit is given, what NBCOT doesn't publish, and how to pace yourself.
Short answer
The NBCOT OTR exam has 180 items in 4 hours at a Pearson test center. Most are single-response multiple-choice items with three or four options and one correct or best answer. The others come in scenario sets: an opening scene followed by four related items, each with six options, of which you select the three best. Scenario sets are mixed in with the other items, and NBCOT doesn't publish how many appear. Some items are unscored field-test items you can't identify. Incorrect choices cost nothing, so answer every item.
The OTR exam gives you 4 hours for 180 items, and since January 2, 2024 those items come in two types: single-response multiple-choice questions and multi-select scenario sets. Everything else about the format, from how credit works to what you can do on screen, follows from those two facts.
The OTR exam at a glance
NBCOT's Foundations page, 2024 Exam FAQ and candidate handbook, checked on October 27, 2026, describe the exam this way:
| Feature | What NBCOT says |
|---|---|
| Items | 180, since January 2, 2024 |
| Item types | Three- and four-option single-response multiple choice, and six-option multi-select scenario sets |
| Exam time | 4 hours. The optional tutorial at the start doesn't count |
| Appointment length | About 4 hours 45 minutes, including check-in, tutorials, surveys and check-out |
| Content | The 2022 OTR content outline: four domains weighted 23%, 23%, 38% and 16% |
| Delivery | Computer-based, in person at Pearson test centers |
| Unscored items | A preselected number of field-test items you can't tell apart from scored ones |
| Scoring | Scaled from 300 to 600, and 450 passes |
The passing score guide explains the scale. The rest of this guide is about what you'll see on screen.
Single-response multiple-choice items on the OTR
Each one has a stem written as a question and three or four options. NBCOT's Foundations page says "there is only one correct or best answer." You click one option, you can change it, you get credit for the correct one, and nothing is deducted for a wrong one. NBCOT notes that some items may include a chart with information needed to answer the question.
NBCOT's handbook examples put qualifiers such as MOST in capitals. A practical habit: slow down on those words, because they turn a question about what is true into a question about what is best for this client.
How OTR scenario sets work
The scenario set is the OTR-only item type, and NBCOT's FAQ, Foundations page and handbook describe it in detail:
- Structure. An opening scene gives general background on a practice-related situation. Four related items follow
- Options. Each item has six options: three correct and three incorrect. The instruction tells you to select the three best choices
- Credit. You "receive credit for each correct response option selected," and there are no deductions for incorrect selections
- No feedback statements. NBCOT's FAQ says the sets have none: the scenario contains all the information you need to answer
- Changing and flagging. You can change answers within an item, flag scenario items, and return to them at any point before you submit
- Placement. Sets are spread through the exam in the same section as the multiple-choice items. They don't come first
How the parts fit together, in a Passy illustration (not an NBCOT item):
| Part | What you see | What to do |
|---|---|---|
| Opening scene | Background on a practice situation, such as a client, a setting and a reason for referral | Read it closely, noting the setting, the client's condition, any precautions and the client's goals |
| Item 1 to item 4 | A question stem and six options for each | Mark exactly three options for each, after checking each one against the scene |
Two points follow from NBCOT's credit rule. Because an incorrect pick costs nothing, marking fewer than three can only lose credit. And because each correct option earns credit, an item where you're sure of two options is still worth finishing carefully.
How many scenario sets are on the exam? NBCOT doesn't say. Its FAQ states that there are "no predetermined item type percentages" across OTR forms, and that every form meets the domain percentages in the outline. A page that gives you a fixed number of sets isn't quoting NBCOT.
NBCOT's Foundations page links two free sample scenario sets. NBCOT notes they "do not replicate the candidate experience on exam day," but they show the layout. Its paid Scenario Item Practice Test has ten sets.
What changed on the OTR exam in January 2024
NBCOT announced the change in July 2023 in OTR Exam Update Coming January 2024, and the FAQ confirms it:
- Clinical simulation (CST) items became scenario sets on January 2, 2024. NBCOT says the sets assess the same knowledge as CSTs in a format that improves accessibility for testing accommodations
- The 2022 content outline took effect for all exams from the same date
- The test vendor changed from Prometric to Pearson VUE, effective January 2, 2024
- The 4-hour limit stayed the same
If a book, course or forum post describes CST problems, it was written for the pre-2024 exam. The content knowledge may still help, but the format advice won't. If you're choosing materials now, you can see how many planned OTR practice questions target Select and Manage Interventions, the 38% domain.
Pacing 180 items in 4 hours
The arithmetic, which is Passy's and not an NBCOT rule: 240 minutes divided by 180 items is 80 seconds per item, or 45 items an hour.
| Time used | Item you should be near |
|---|---|
| 1 hour | 45 |
| 2 hours | 90 |
| 3 hours | 135 |
| 4 hours | 180 |
At that pace, a four-item scenario set gets about 5 minutes 20 seconds, including the opening scene. Two rules from the handbook affect your pacing: breaks are allowed, but "the exam clock will continue to run," and the Help screens are available during the exam, also on the clock. A practical approach is to answer every item on the first pass, flag the ones you want to revisit, and leave a few minutes at the end for flagged items.
Full OTR prep is planned with a 180-question mock exam of single-answer questions, taken as one timed 4-hour sitting and scored at the end, so you can rehearse the 4-hour OTR clock with the planned timed mock exam once it opens. Checkout isn't open yet.
On-screen tools and rules during the OTR exam
From NBCOT's Foundations page and handbook:
- Highlight and strike out. Highlight text you want to come back to, and strike out options you've ruled out
- Flag for review. You can flag items and change answers until you submit or time runs out. When time runs out, your selected responses are submitted. A flagged item with no option selected gets no credit
- Display. You can change the background and text colors and zoom the content up to 200%
- Note board. You can ask test center staff for a wet-marker note board. Taking notes isn't allowed during the tutorial, the acknowledgment or before the exam timer starts
- Acknowledgment. You must accept its terms within seven minutes before the exam launches, or the exam closes and the appointment is forfeited
NBCOT's Take the Exam page links the OTR exam tutorial on Pearson VUE's site, and Pearson VUE's NBCOT page also links it. Running it once before test day takes the surprises out of the interface.
OTR and COTA formats are not the same
Search results often mix the two NBCOT exams. The handbook keeps them apart:
| OTR | COTA | |
|---|---|---|
| Items | 180 | 190 |
| Multi-select format | Scenario sets: a scene plus four six-option items | Standalone six-option items |
| Domains | 4 (23%, 23%, 38%, 16%) | 3 (27%, 55%, 18%) |
| Time | 4 hours | 4 hours |
If a resource says 190 items or three domains, it's describing the COTA exam.
Putting the OTR format to work
Before you practice content, get used to the screen. Take the OTR exam tutorial and the two free sample scenario sets, then do a short timed set at 80 seconds an item. The 20 free OTR practice questions are original single-answer items with explanations, a quick way to test your pace on that item type; they don't include scenario sets. The study plan guide shows where timed practice fits in a 6- or 12-week schedule, and the exam page lets you review the OTR exam facts and official NBCOT links in one place.
