NBCOT OTR Study Plan: Weighting the Four Domains in 6 or 12 Weeks
NBCOT's Prepare page says students report spending 6 weeks studying for the exam. Here is how to turn the 2022 OTR outline's 38/23/23/16 weights into study hours, a 6-week and a 12-week schedule, how to use NBCOT's single-use practice tests, and the study rules in the Code of Conduct.
Short answer
NBCOT's Prepare page says students report spending 6 weeks studying for the exam, and NBCOT suggests building your plan from the content outline. Weight your time by the 2022 OTR outline: Select and Manage Interventions 38%, Evaluation and Assessment 23%, Analysis, Interpretation, and Planning 23%, and Competency and Practice Management 16%. Six weeks suits a recent graduate who can study most days. Ten to 12 weeks suits someone working, returning after a gap, or retaking. Either way, finish with timed practice for 180 items in 4 hours, including scenario sets.
A good OTR plan answers two questions: how many weeks, and how to split them. NBCOT gives a starting point for the first and a formula for the second.
How long NBCOT candidates study
NBCOT's Prepare page, checked on October 27, 2026, says: "Students report spending 6 weeks studying for the exam." It's a report of what students do, not a requirement. The same page lists five steps: choose your test date, create a study plan that includes time for self-care, review the content outline, take a self-assessment, and use study tools from a trusted source. NBCOT's Exam Outline page adds that you can use the outline "to build your study plan."
Six weeks is realistic if you've just finished fieldwork and can study most days. A longer runway, 10 to 12 weeks, makes sense in these situations:
- You're working and can study only evenings or weekends
- Fieldwork skipped whole areas, such as pediatrics, mental health or hand therapy
- You graduated a while ago and coursework feels distant
- You're retaking and need to change your method, not just repeat it. The retake guide covers the waiting periods
The total matters more than the calendar. Six weeks at 15 hours a week is 90 hours; twelve weeks at 8 hours is 96. Pick the version you can actually keep.
Turning the OTR domain weights into study hours
The 2022 OTR content outline weights the four domains. Per 100 hours of study, the split is simple:
| Domain | Weight | Hours per 100 | Roughly this many of 180 items |
|---|---|---|---|
| Select and Manage Interventions | 38% | 38 | 68 |
| Evaluation and Assessment | 23% | 23 | 41 |
| Analysis, Interpretation, and Planning | 23% | 23 | 41 |
| Competency and Practice Management | 16% | 16 | 29 |
The item counts are Passy's approximations: NBCOT says every form meets the domain percentages, but each form also includes unscored field-test items in a number it doesn't publish. A practical adjustment: shift a little time toward the domain where your practice results are weakest, but don't drop any domain, since all four count toward one total score. To keep each domain's recall fresh during the content pass, you can work through 470 flashcards sorted by OTR domain once full prep opens.
What each OTR domain asks you to study
The outline has 16 tasks and 62 knowledge statements. Its examples, quoted or summarized here, show where the hours go:
| Domain | What it covers, from the outline's tasks and examples |
|---|---|
| Evaluation and Assessment (3 tasks) | Development and aging, conditions and their progression, lived experience and identity; standardized and nonstandardized tools, including criterion-referenced and norm-referenced tests, interviews and observation; activity analysis |
| Analysis, Interpretation, and Planning (3 tasks) | Interpreting quantitative results (statistical terminology, scoring) and qualitative findings such as the occupational profile; interprofessional plans, including IEP, discharge, transition, and palliative and hospice care; health literacy; monitoring and modifying plans; precautions such as postsurgical precautions, vital signs, laboratory values and suicidal ideation |
| Select and Manage Interventions (6 tasks) | Preparatory methods and physical agent modalities, wound management; occupation-based interventions for ADL, IADL, play, leisure, work and groups, cognition and perception, eating and swallowing, transfers, and wellness and mental health; therapeutic exercise, edema and scar management, constraint-induced movement; orthoses and prostheses; assistive technology, seating and mobility, community mobility; environmental modifications and universal design |
| Competency and Practice Management (4 tasks) | Research appraisal and quality improvement; population health such as fall prevention and aging in place; infection control and emergencies such as seizures and diabetic reactions; laws, state practice acts, supervision, reimbursement and documentation; professional development and ethics under NBCOT's Practice Standards and Code of Conduct |
Read the full outline once at the start and mark each knowledge statement as strong, shaky or unfamiliar. That list becomes your study order inside each domain.
A 6-week NBCOT OTR study plan
An editorial template, built so the content weeks roughly follow the domain weights and the last two weeks are mostly practice:
| Week | Main focus | Practice |
|---|---|---|
| 1 | Baseline: a timed mixed set, the outline self-rating, the OTR exam tutorial. Then Select and Manage Interventions: occupation-based strategies and sensory, motor and neurological interventions | Short sets on each topic as you finish it |
| 2 | Select and Manage Interventions: modalities, wound care, orthoses and prostheses, assistive technology, seating and mobility, environmental modifications. Then start Evaluation and Assessment | Timed sets of 20 to 30 items |
| 3 | Evaluation and Assessment, then Analysis, Interpretation, and Planning | Timed mixed sets, plus NBCOT's two free sample scenario sets |
| 4 | Finish Analysis, Interpretation, and Planning, then Competency and Practice Management | A full-length timed rehearsal at the end of the week |
| 5 | Review by domain from your practice results, weakest first | Timed mixed sets with scenario practice |
| 6 | Early in the week, a last long timed practice. Then light review of recent misses and test-day logistics: ID, route, ATT dates | Short sets only in the last two or three days |
If you study from a single question bank, it helps to practice one domain at a time in the full OTR prep planned for launch during weeks 1 to 4, then switch to mixed sets.
Stretching the OTR plan to 12 weeks
The same structure, with a longer content pass and room for a second full rehearsal:
| Weeks | Focus |
|---|---|
| 1 | Baseline, outline self-rating, exam tutorial |
| 2 to 4 | Select and Manage Interventions |
| 5 and 6 | Evaluation and Assessment |
| 7 and 8 | Analysis, Interpretation, and Planning |
| 9 | Competency and Practice Management |
| 10 | Mixed timed practice and the first full-length rehearsal |
| 11 | Weakest domains, then a second full-length rehearsal |
| 12 | Light review, recent misses and logistics |
Keep about a quarter of each week for mixed questions from domains you've already covered, so early material doesn't fade by week 12. The free OTR study planner builds a weighted outline from your exam date and daily minutes if you'd rather not lay it out by hand.
Using NBCOT's study tools without wasting them
NBCOT sells its own Aspire tools, and its study tools page says they're built on the same validated domain, task and knowledge statements as the exam. What to know before you buy or use them:
- Practice tests. NBCOT's practice tests page lists OTR Practice Tests 1 to 3, with 110 questions each, and a Scenario Item Practice Test with ten scenario sets. Each is "designed for one-time use," gives a scaled score report, and doesn't show which answers you got wrong. You don't have to finish one in a single sitting
- The StudyPack. NBCOT's StudyPack page lists a full practice exam, practice tests, mini tests with rationales and references, flashcards, Knowledge Match, an electronic study guide and a study planner. The handbook lists it at $85. The pre-test, practice tests and full practice exam are single-use, access lasts until you pass, and you get a fresh StudyPack after an unsuccessful attempt
- Free materials. The outline, the OTR exam tutorial (linked from NBCOT's 2024 Exam FAQ) and the two sample scenario sets on NBCOT's Foundations page. The Foundations page also lists textbooks NBCOT may use to check the accuracy of items, while noting that items aren't written from textbooks and that using them doesn't guarantee a pass
A practical rule for single-use tests: don't spend them on day one. Take one around the middle of your plan to check direction, and keep one for the final rehearsal. NBCOT also notes that it "does not require, approve, or endorse" any preparatory courses or study materials, and that includes Passy's.
The study rules in NBCOT's Code of Conduct
Principle 9 of the Code of Conduct, printed in the handbook, bars discussing or disclosing NBCOT exam content and "copying, uploading, downloading or sharing NBCOT study tool" content "through any platform including web-based study tools, AI applications, social media outlets, discussion forums, email." The handbook warns that violations can jeopardize your ability to sit for the exam or be certified, and NBCOT's Prepare page asks every candidate to watch its video on cheating.
In practice:
- Avoid "recalled question" collections and flashcard decks that copy NBCOT practice-test items
- Don't paste NBCOT study-tool questions into AI tools or share them with classmates
- Keep study groups to concepts and original questions, never items recalled from the exam
Passy's questions are written from the outline and contain no NBCOT exam items or study-tool content.
Practicing for scenario sets and the 4-hour OTR clock
Content knowledge isn't enough on its own. In the last third of any plan:
- Time everything at 80 seconds per item, which is 4 hours for 180.
- Practice "select three" items with NBCOT's sample sets and, if you buy it, the Scenario Item Practice Test. The format guide explains how they earn credit.
- Do at least one full-length rehearsal in a single sitting, with breaks counted against the clock, as they are on test day.
- Review every miss until you can say why each wrong option is wrong, then log the topic under its domain.
For the full-length piece, you can add the planned 180-question OTR mock exam to the last two weeks of your plan once it opens; its questions are single-answer, so keep NBCOT's sample sets for scenario practice. Today, start with the 20 free OTR practice questions, which are single-answer items with explanations, as your week 1 baseline, and see the difficulty guide for why that baseline matters.
