Study therapeutic communication, contributing to a safe environment for a client in crisis, asking directly about suicide when cues appear, and reporting abuse or unsafe behavior to the RN. Do not treat PN items as an invitation to independently diagnose or start a psychiatric medication change.
PN weights this category more than current RN
The 2026 PN range for Psychosocial Integrity is 9–15%. The 2026 RN range is 6–12%. That is one reason “LPN mental health NCLEX” is a real search. Practice psychosocial items as stay, ask, and report.
You still do not independently write a treatment plan for major depression. You communicate, observe, keep the environment safer, and escalate.
Communication patterns that usually win
- Open, specific, and tied to the client’s words.
- Direct suicide question when the client talks about being a burden or “better off dead.”
- No “don’t feel that way,” no subject change to meds only, no leaving the person alone.
Original example: “My family would be better off without me.” Best LPN action: ask about self-harm, stay, notify the RN. Coordinated Care and psychosocial both apply.
Abuse or neglect cues (child, elder, intimate partner) are report items. The LPN does not delay for a family meeting “to be sure.”
What not to memorize as a DSM dump
You may see anxiety, grief, substance withdrawal, or cognitive changes. The testable action is still: safety, orientation, monitoring withdrawal per protocol, notifying the RN of escalating CIWA-type findings—not independently starting a benzodiazepine taper.
