NBCOT OTR Exam Cost: $540 to Apply, $430 to Reapply, and the Fees People Miss
The OTR exam costs $540 with the online application and $430 for each reapplication, as checked on October 27, 2026. Here is every NBCOT fee around it, from $35 services to the $115 ATT reissue, what is refundable, and worked budgets for a first attempt and a retake.
Short answer
The NBCOT OTR exam costs $540 with the online application, as checked on October 27, 2026, and $430 each time you reapply after an unsuccessful attempt. A paper application costs $595. One Score Transfer and one Eligibility Confirmation Notice are free if you order them with the application, and later or extra requests are $35 each. Arriving late or without proper ID, changing an appointment within 48 hours, or missing it costs $115 to reissue your Authorization to Test. State license fees are separate.
Budget $540 for the exam itself if you apply online. The extra costs come from three places: services you forget to add at checkout, appointment mistakes, and a retake. All of them are predictable.
NBCOT OTR exam fees as of October 27, 2026
From NBCOT's apply page, candidate handbook and eligibility page:
| Fee | Amount |
|---|---|
| Exam application, online | $540 |
| Reapplication after an unsuccessful attempt, online | $430 |
| Exam application, paper | $595 |
| Score Transfer to a state board, beyond the free one | $35 each |
| Eligibility Confirmation Notice, beyond the free one | $35 each |
| ATT reissue after late arrival, missing ID, a change within 48 hours or a no-show | $115 |
| Duplicate MyNBCOT account | $35 |
| Processing fee on any refund | $40 |
| OTED eligibility review, for internationally educated therapists | $850 |
NBCOT's apply page warns that a paper application increases processing time. The current paper form (ID 37 rev053124) also lists a paper reapplication fee of $485. NBCOT's apply page and handbook list no member, student or promo discount on the exam fee.
What the $540 covers
The $540 is the exam application fee, and once NBCOT approves the application it lets you take the exam within your Authorization to Test (ATT). NBCOT lists no separate charge for the character review or its background check. You also get two free services, but only if you add them before paying:
- One Score Transfer, which sends your result to a state board. NBCOT's Score Transfer page says every state except Hawaii requires one to issue a license
- One Eligibility Confirmation Notice, which confirms to a state board, an employer or yourself that you're approved to test. Candidates typically use it for a temporary permit, though the handbook lists 15 states that don't accept it
After you pay, each request costs $35, including the first one if you skipped it at checkout. Ordered services can't be canceled or refunded, and a Score Transfer to one state can't be swapped for another. If the Social Security number you gave NBCOT doesn't match the one your state board has, the state can't access your result and you have to buy a new transfer.
What isn't included: study materials, your state license application and its fee, any ATT reissue, and a retake.
Worked budgets for an OTR first attempt and retake
These are Passy's sums from NBCOT's published fees. State license fees are left out because they vary by state.
| Scenario | NBCOT fees |
|---|---|
| Apply online and add the free Score Transfer | $540 |
| Same, but order the Score Transfer after paying | $575 |
| Score Transfers to two states, both added at application | $575 (one free, one at $35) |
| One no-show, then a reissued ATT on the same application | $655 |
| An unsuccessful first attempt, then one online reapplication | $970 |
| Two reapplications | $1,400 |
Each new application includes its own free Score Transfer and Eligibility Confirmation Notice if you add them when you reapply, because the free services don't carry over. NBCOT notes that some states require a Score Transfer for every exam attempt, so add one each time unless your board says otherwise. The OTR retake guide covers the waiting periods that come with a reapplication. If you're also budgeting for prep, you can compare the one-time prices planned for 30, 90 and 180 days of OTR prep. The $115 mistakes, and how to avoid them
The handbook charges $115 to reissue an ATT in four situations:
- You arrive more than 15 minutes late and can't test. NBCOT asks you to arrive 30 minutes before the appointment.
- You arrive without proper ID and can't test. You need two original, unexpired IDs with signatures, and the first and last names must match your ATT exactly.
- You cancel or reschedule less than 48 hours before the appointment. The handbook lists no fee for changes made earlier through MyNBCOT.
- You don't show up.
Two more rules make these worse. A reissue is only possible while your application is still active, within 90 days of submitting it. Once the application expires, the ATT can't be reissued, and you must submit a new application with the full fee. And not accepting the on-screen acknowledgment within seven minutes forfeits the appointment, with a fee to reissue the ATT. For an emergency inside 48 hours, NBCOT asks you to contact credentialingservices@nbcot.org or 301-990-7979.
Refunds, withdrawals and expired OTR applications
NBCOT's refund policy is short: no refunds, returns or exchanges, except for duplicate payments, overpayments and exam application withdrawals, and a $40 processing fee applies to every refund.
- Withdrawing. Email info@nbcot.org with a brief explanation before your application expires. A partial refund may be granted, and approved refunds are processed within 30 days. You can't reapply for three months
- Emergencies. If a personal or medical emergency stops you from testing while your application is active, write to credentialingservices@nbcot.org. NBCOT considers these case by case and may ask for documentation
- Letting it expire. The paper application states that if you don't test within your eligibility period, all fees are forfeited
NBCOT study tool prices
NBCOT sells its own study tools, called Aspire. The handbook lists:
| Tool | Price |
|---|---|
| Exam StudyPack | $85 |
| Study Guide | $60 hard copy, $40 electronic |
| Flashcards | $15 |
| Knowledge Match game | $9 |
NBCOT's StudyPack page says access lasts until you pass, and that after an unsuccessful attempt you get a fresh StudyPack, so its pre-test and practice exams can be taken again. Practice tests are sold separately or inside the StudyPack, and their prices aren't listed on the pages checked for this guide. NBCOT also says it "does not require, approve, or endorse" any prep courses or materials, and that its own tools don't guarantee a better result. If you're comparing options, you can see which study features full OTR prep plans to include, from flashcards to Fix It, alongside NBCOT's tools.
Costs after you pass the OTR
Two costs come after the exam, and neither is on NBCOT's exam fee list:
- Your state license. Every jurisdiction sets its own application and fee, and NBCOT stresses that certification and licensure are separate. NBCOT's state board directory links each board
- Certification renewal. Your first NBCOT renewal comes four years after the year you pass, between January and March, and requires 36 units plus a renewal fee. The handbook doesn't state the renewal fee
Before you apply, list the services you'll need at checkout, especially the Score Transfer for your state, so you don't pay $35 for something that was free. Then build your preparation around the date: the free OTR study planner sets a weekly schedule, and the application guide walks through each step. Before full prep launches, you can check what every planned OTR access period includes.
