How Many Questions Are on the NREMT Paramedic Exam? 110 to 150 in 3.5 Hours
Every NREMT Paramedic candidate answers at least 110 items and at most 150, with 3.5 hours on the clock. Here is why the count varies and how to pace the time.
Short answer
The National Registry Paramedic Certification Examination gives every candidate at least 110 items and at most 150, and you have 3.5 hours to finish. Twenty of the items are unscored pilot questions that you cannot identify, so between 90 and 130 of the items you see count toward your result. The exam is computer adaptive: it can stop at 110 once it reaches a pass or fail decision with 95% confidence, or keep going until you reach 150 items or run out of time.
NREMT Paramedic item count and time limit at a glance
The National Registry Paramedic Examination Specifications, in use since July 1, 2024, and the Paramedic Candidate Handbook give the same numbers. The scored-item row is Passy's subtraction. Everything else comes straight from those two documents.
| Rule | NREMT Paramedic exam |
|---|---|
| Minimum items | 110 |
| Maximum items | 150 |
| Unscored pilot items | 20, included in the count above and not marked |
| Items that count toward your result | 90 if the exam stops at 110, up to 130 at 150 (Passy's arithmetic) |
| Time limit | 3.5 hours |
| Format | Computer adaptive test (CAT) |
| Where you take it | Pearson VUE test centers, according to the test plan |
The 20 pilot items are there so the National Registry can collect data on new questions before using them as scored items. They do not affect your score, and nothing on screen tells you which ones they are. The handbook's advice is the only sensible one: answer every item as if it counts.
Why your NREMT Paramedic exam might stop at 110 or run to 150
The count varies because the exam adapts to you. After an initial set of items, the computer starts choosing items targeted at or above its current estimate of your ability. The handbook puts it plainly: the more questions you get right, the harder the exam gets. So the exam can feel hard even while it is going well.
The test can end in one of three ways:
- At the minimum. If it can make a pass or fail decision after 110 items with 95% confidence, it stops.
- At the maximum. If it cannot decide yet, it keeps giving items, up to 150.
- At the time limit. It also stops when the 3.5 hours run out.
The passing standard is the same for every candidate, whatever the length of their exam. The 95% figure is a confidence level for the decision, not a percentage of correct answers. If you are reading this after your exam, the 110 or 150 questions guide explains why the length tells you nothing about the result.
The National Registry uses the same adaptive method for its EMT exam, and Passy's EMT guide on how the NREMT adaptive test works explains it in more depth. Only the method carries over: the EMT numbers differ, with 70 to 120 items, 10 unscored pilots and 2 hours.
Pacing 3.5 hours across up to 150 NREMT Paramedic items
You cannot know in advance whether your exam will stop at 110, so pace for 150. The arithmetic:
- 3.5 hours is 210 minutes, or 12,600 seconds.
- 12,600 seconds ÷ 150 items = 84 seconds per item if the exam runs to the maximum.
- 12,600 seconds ÷ 110 items = about 114.5 seconds per item if it stops at the minimum.
The checkpoints below are a Passy pacing plan at 84 seconds per item, rounded to whole items. They are a study habit, not a National Registry rule.
| Time used | Items finished at an 84-second pace |
|---|---|
| 30 minutes | about 21 |
| 1 hour | about 43 |
| 1.5 hours | about 64 |
| 2 hours | about 86 |
| 2.5 hours | about 107 |
| 3 hours | about 129 |
| 3.5 hours | 150 |
Items will not take equal time. A scenario-based set asks several questions about one patient, and build list or drag-and-drop items can take longer to answer than a multiple-choice item. Bank a few seconds on the quick multiple-choice items so the slower formats do not push you behind a checkpoint.
Twenty items at 84 seconds each comes to 28 minutes. Set a 28-minute timer and time yourself on 20 NREMT Paramedic questions at an 84-second pace to feel the rhythm. The Passy set is a fixed, single-answer set, so it trains pace and reasoning, not the adaptive experience or the interactive formats.
The NREMT Paramedic seat timeline: agreement, tutorial, then 3.5 hours
The 3.5 hours do not start the moment you sit down. The handbook describes this order at the workstation:
| Stage | Time allowed | What to know |
|---|---|---|
| Nondisclosure agreement | Up to 5 minutes | If you do not finish it in time, you cannot begin. It counts as an attempt and you forfeit the fee. |
| Tutorial on the item types | 10 minutes | Instructions on how to respond to each item type, before the exam clock starts. |
| The exam | 3½ hours | 110 to 150 items. |
That is up to 3 hours and 45 minutes at the computer, plus check-in time at the test center. Read the agreement, but keep an eye on its 5-minute limit. The exam day guide covers check-in and the test-center rules, and the question types guide explains the formats the tutorial shows.
Why other NREMT Paramedic question counts circulate
If a prep site or a classmate quotes different numbers, check which exam and which date they mean. Three common sources of confusion:
- Another level. The EMT exam has its own specification, administered since spring 2025, with a 70-item minimum, a 120-item maximum and 2 hours. The National Registry's application page also lists the AEMT exam as computer linear rather than adaptive.
- An older Paramedic exam. Figures for the exam given before July 1, 2024 do not describe the current one. Use a source dated after the July 2024 launch that gives 110 to 150 items and 3.5 hours.
- An older test plan file. An earlier version of the Paramedic test plan, still opened by the handbook's sample-items link, states the 110-item minimum but leaves out the sentence on a 150-item maximum. The July 2024 test plan and the handbook both give 110 to 150.
The six-domain guide shows how those 110 to 150 items divide across the content.
