NREMT Paramedic Exam Cost: $175 Per Attempt and the Other Fees

The National Registry charged $175 for each Paramedic exam attempt as checked on October 18, 2026. Here are the other fees, the refund rules, and what common situations cost.

Short answer

As checked on October 18, 2026, the National Registry charges a $175 application fee for each Paramedic Certification Examination attempt, so a retake costs another $175. Rescheduling or cancelling through Pearson VUE costs $30, more by phone, and a rescoring request costs $150. Missing your appointment without at least one business day's notice, or letting your 90-day ATT expire, forfeits the fee, and once you are certified, recertification costs $37 for applications from October 1, 2026.

NREMT Paramedic fees as checked on October 18, 2026

The National Registry charges for the Paramedic Certification Examination per attempt, not per certification. Every amount below comes from the Paramedic Candidate Handbook, the National Registry's examination and pathway pages, or its recertification page, as checked on October 18, 2026. Fees can change, so confirm them in your account before you pay.

FeeAmountWhen it applies
Application fee$175Each examination attempt, including every retake
Reschedule or cancel through Pearson VUE$30Each change made online. Pearson adds a call-center fee for changes by phone; the handbook does not list that amount
Rescoring request$150Paid before the review. Request within 30 days of the exam date. Refunded if the review finds an irregularity
New ATT after expiry or a no-show$175You pay the application fee again for a new attempt
Recertification fee$32 through September 30, 2026, $37 from October 1, 2026Every two years after you are certified
Late recertification application$50Recertification applications filed after the March 31 deadline and by April 30, with all education finished by March 31

For comparison, the National Registry's EMT exam costs $104 per attempt on its own pathway page. The Paramedic fee covers one exam on one date, with no separate psychomotor exam since July 1, 2024.

What common NREMT Paramedic situations cost

These totals add only the National Registry and Pearson fees listed above. The scenarios are Passy's examples.

SituationWhat you payTotal in exam fees
Pass on the first attemptOne application fee$175
Reschedule once online, then passApplication fee plus one Pearson change$205
Miss the appointment without 24 hours' noticeThe first fee is forfeited, then a new application$350, unless a no-show appeal succeeds
Let the ATT expire without testingThe first fee is forfeited, then a new application$350
Fail once, pass on the retakeTwo application fees$350
Fail once, request rescoring, then retakeTwo application fees plus rescoring$500, less $150 if the review finds an irregularity

Two of those rows are avoidable with a calendar. The ATT is valid for up to 90 days, and Pearson must hear from you at least one business day (24 hours) before the appointment, or the account is marked as a no-show and the fee is lost. A no-show can be appealed with documentation of extenuating circumstances, and a successful appeal lets you reschedule the missed exam at no additional cost.

A retake also has a waiting period. The handbook says to wait 15 days from your last examination date, while retesting policy 62.10 says you can schedule 15 days from the day your results post, counting that day as day 1. Use the eligible date in your account. After three failed attempts you must also complete remedial education before a fourth attempt, which may carry its own course cost. The retake policy guide explains both rules.

When the National Registry refunds the Paramedic fee

The handbook allows refunds within 90 days of payment, and only for three reasons:

  • you completed the wrong application
  • you no longer want National Registry certification
  • you are already certified and decided not to recertify by examination

The National Registry does not refund fees for failing to appear at a scheduled appointment. A failed attempt is not a refund reason either. You pay $175 for the next attempt, as the passing score and results guide explains in its section on next steps.

Fees after you pass: recertification

National Registry Paramedic certification is renewed every two years. Your first expiration date is March 31, two or three years after you complete the certification process, depending on whether you finish in the first or second half of the year. On October 18, 2026, the Paramedic recertification page listed a $32 recertification fee through September 30, 2026 and $37 from October 1, 2026.

Late recertification has two $50 charges, depending on the case. If you finished your education by March 31 but miss the application deadline, you can pay a $50 late application fee and apply until April 30. The page also describes reinstatement until April 30 for the same situation, with a $50 reinstatement fee added to the standard recertification fee. If you choose to recertify by examination instead of continuing education, the page tells you to pay the examination fee, and it lists that fee as $175 per attempt. The recertification guide covers the timeline.

Costs this NREMT Paramedic guide leaves out

Several real costs are not National Registry fees, and they vary too much to quote:

  • Paramedic program tuition, set by your program.
  • State license fees, set by your state. National Registry certification is not a license to practice, so the state license is a separate step with its own rules.
  • Remedial education after three failed attempts, priced by whoever offers the course.
  • Travel to a Pearson VUE test center, which depends on where you live.

Budget for these separately. For the order of payment and scheduling steps, see the application guide.
